Brief description of study

This is a multicenter, randomized, quadruple-blind, sham controlled, 2-part, embedded IDE clinical trial. The main objective is to evaluate the efficacy and safety of Cardiac Contractility Modulation (CCM) therapy in patients with symptomatic HF with LVEF 40-60%. The study is divided into 3 phases, screening, randomization (where 2/3 will be randomized to CCM, 1/3 to sham procedure and long term (unblinded) follow-up visits where where subjects on sham procedure will have CCM turn on.
